Have things gone wrong in space? Unfortunately, yes. Space is a dangerous place and the spacecraft we send there do occasionally stop working. we try really really hard to stop this and fix them when they do break.

Sometimes, things that go wrong on spacecraft can be dangerous to astronauts or people working on the spacecraft. There are, sadly, a few times that people have died because things went wrong, such as the Challenger shuttle disaster, the Apollo 1 fire or Soyuz 11.

Sometimes we run out of coffee.

Space is a very dangerous place and very different from what we are used to and what we design most of our tools and equipment for. While space environment is fatal to humans (and Alex mentioned some examples of things going wrong) it also effects our equipment. Many unmanned missions failed but there were many more great success.

Oh yes, thats we do so much testing on the ground. One of the pictures in my profile is of large loudspeaker like device that we use to test parts can survive being launched into space as they are shaker on the rocket.

We always design things to survive this, but then we need to test that they really will. Occasionally we discover something during testing, so we go back, fix the design and test again. Its better to have it break on the ground where we can fix than when it is in space on the satellite.

This is also why putting people into space is so much more complicated than launching robots. We have to be really sure that everything will work properly before risking putting people on a rocket into space.

I have had lots of challenges on projects but luckily nothing major has gone wrong.
